If you prefer to have a phlebotomist collect your samples—still just 2 small vials—choose any certified phlebotomist. A few suggestions below. Book and pay them directly.

- Home visit options: TravaLab, The National Phlebotomy Provider Network, Phleb-Finders, and Speedy Sticks.
For samples not yet sent due to insufficient volume or missed same day shipping:
- Partial Replacement Kit for Self-Collection: $60
- Or Partial Replacement kit for Phlebotomy: we'll take the loss by sending the kit at no cost. You'd just pay the phlebotomist directly.
For samples that have been sent but are not usable:
- Both vials are not not usable: Full Replacement kit for Self-Collection: $100
- One of the two vials is not not usable: Partial Replacement Kit for Self-Collection: $60
- Or Full Replacement Kit for Phlebotomy: $60 + you pay phlebotomist directly.
When shipping delays affect your samples:
- Carrier delays: Full Replacement Kit for Self-Collection due to Delayed Delivery for free.
- Missed drop-off time or drop-off at non-FedEx Office location: Full Replacement kit for Self-Collection: $100
